Chronic Disease Management
You are eligible to join this programme free of charge if you:
Are over the age of 18
Have a Medical Card or GP Visit Card
Have one or more of these chronic conditions:
Asthma
Chronic Obstructive Pulmonary Disease (COPD)
Type 2 Diabetes
Cardiovascular Disease (including Heart Failure, Angina/Heart Attack, Stroke, Irregular Heartbeat/Atrial Fibrillation)
How will I benefit from participating in the programme?
The programme aims to help you manage your condition and prevent hospital admissions if possible through:
structured reviews of your chronic disease with your GP or Nurse
a personalised care plan of your needs developed and agreed with your GP
regular reviews of your medications and care plan
opportunities for structured education and self management support
early detection of any new condition that you may develop
early detection of any complications in your condition
care in your community, close to your home
What do I need to do next?
Your GP or Nurse will invite you to register for the Structured Chronic Disease Management/CDM Programme.
You can also contact us by phone or email to let us know if you would like to participate.
The programme includes 2 free review in every 12 month period that you are part of the programme.
Each review includes one visit with the practice nurse followed by a visit with your GP. These reviews may be in the GP practice or over the phone. Your visit may include tests such as blood tests. You may be advised to have other tests such as an ECG or 24 hour Blood Pressure Monitor.
There is no charge of any tests carried out as part of your programme reviews.
You will receive a written care plan after each review.
You can continue to visit your GP as normal outside of the scheduled CDM reviews.

As part as part of the programme, your GP or Nurse will record some information about you at each review.
This will include your
Name & Age
Chronic Disease Diagnosis
Medical History
Details of any symptoms or tests you have had since your last visit
Why is my information useful?
Information gathered as part of the CDM Programme will play a vital role in helping to improve our understanding of Chronic Disease.
It will improve the Health Services ability to detect, treat and prevent chronic diseases, as well as deliver an improved service to those with chronic diseases.
